California Cardiovascular Prevalence Data by County and City

This page publishes modelled prevalence estimates for seven cardiovascular and metabolic conditions across
every county in California and 1,522 California cities and places. The figures come from the CDC’s PLACES programme
and are reproduced here in full, with the complete file available to download.

What is in the data

12,824 rows covering 58 counties and 1,522 cities, for the 2023 release. Each row carries the estimate, the lower
and upper 95% confidence limits, the total population and the adult population of that place.

The seven measures are coronary heart disease, stroke, diagnosed diabetes, high blood pressure, high cholesterol
among adults who have been screened, obesity, and current cigarette smoking. The file also carries the share of
adults with high blood pressure who report taking medication to control it.

How to read these figures

These are modelled small area estimates, not direct measurements. The CDC produces them by applying a
statistical model to responses from the Behavioral Risk Factor Surveillance System, combined with census
population data. They describe the estimated prevalence among adults in a place. They are not a count of diagnosed
patients, and they carry the uncertainty shown in the confidence limits published alongside each one.

One limitation matters for anyone arriving here looking for vascular data specifically. PLACES does not include a
measure for peripheral artery disease. The closest available indicators are coronary heart disease, diabetes and
stroke, which share risk factors with it but are not the same condition.

What the spread looks like

Across California counties, estimated coronary heart disease among adults ranges from 4.5% in Santa Clara to 11.0%
in Trinity. At city level the range is wider still, from 0.8% to 18.2%, which reflects how strongly these conditions
track with the age profile of a place. The highest city figures in the state belong to retirement communities.
